Orthopedic and Muscle Testing for the Acupuncturist - Course 2
by Derrick McBride, Whitfield Reaves
In this course, we will discuss orthopedic and muscle tests of the cervical spine, glenohumeral joint, elbow, wrist and hand.
Orthopedic and Muscle Testing for the Acupuncturist - Course 2
Course Overview
This course will provide the acupuncturist with the clinical skills to test for orthopedic conditions appropriately and accurately. The orthopedic and manual muscle tests selected for this training are easy to implement for any acupuncturist. And they are relevant to the clinic environment, regardless of previous orthopedic background. The instructors, Derrick McBride and Whitfield Reaves, believe understanding the anatomical nature of orthopedic conditions is the first step to precisely treating pain and injuries. These courses are organized to be viewed in order, as an entire series Orthopedic and Muscle Testing for the Acupuncturist. We strongly recommend that the practitioner complete the courses of this series from beginning to end.
In Course 2, Derrick McBride lectures on the proper way to perform the test and possible positive findings for each test. High definition videos of each test are used to demonstrate. Helpful tips on positioning, cueing, and other clinical tricks to ensure reliable testing conditions are emphasized. The instructor often comments on clinical relevancy and real-life applications, especially for the Acupuncturist.
Course 2 covers Orthopedic and Muscle Tests of the Upper Extremity. The following topics are covered:
(1) TMJ
(2) Cervical Spine Nerve Root Involvement
(3) Cervical Musculature
(4) AC Joint Syndromes
(5) Glenohumeral Joint Syndromes
(6) Glenohumeral Joint Musculature
(7) Elbow Structure Tests
(8) Elbow Inflammatory Syndromes
(9) Elbow Musculature Tests
(10) Wrist and Hand Nerve Entrapment
(11) Wrist Inflammatory Syndromes
Whitfield Reaves' series Mastering the Treatment of Injury and Pain covers in depth techniques for treatment of the conditions. Students should consider using these series together for a complete understanding of assessment and treatment of orthopedic conditions. Courses 1 through 4 of Whitfield's Mastering the Treatment of Injury and Pain correspond to the topics covered in this course, the Upper Extremity.

Course Objectives
- Participants will understand how orthopedic and muscle testing should be utilized in an acupuncture setting. An in depth knowledge of when to perform the different tests and how that can help drive treatment strategy will be emphasized.
- Participants will feel comfortable with appropriate positioning and cuing for all tests covered. Specific instructions on how to best instruct the patient to produce force or resist force given the test, will be explicitly outlined.
- Participants will be able to accurately and effectively conduct orthopedic and muscle testing for the upper extremity. The student will be taught the tests using verbal instruction, written instruction, and visual instruction.
Course Outline
0 hrs - 45 minThe first section begins with an introduction of orthopedic and muscle tests. Then testing for the TMJ and Cervical Nerve Root compression is covered.
45 min - 1.25 hrsMuscle tests for the cervical spine including the longus colli, upper trapezius, splenius capitus, splenius cervicis, and levator scapulae are covered. Proper positioning and methods for completing the tests is emphasized.
1.25 hrs - 2.25 hrsOrthopedic tests for the glenohumeral joint and AC joint are covered. Arc of Pain, Speeds Test, Cross Over Impingement, Hawkins Kennedy, Posterior Impingement, Piano Key Sign, Sulcus Sign, Grind Test, Jobe Relocation Test, and Roos Test are covered.
2.25 hrs - 2.75 hrsGlenohumeral muscle tests of the supraspinatus, Infraspinatus, Subscapularis, Anterior Deltoid, and the Lateral Deltoid are covered. Proper positioning and methods for completing the tests is emphasized.
2.75 hrs - 3.25 hrsOrthopedic tests for the elbow including Tennis Elbow, Golfers Elbow, Elbow Hyperextension, Elbow Flexion, Elbow Valgus, Elbow Varus, and Tinels Sign are covered. Proper positioning and methods for completing the tests is emphasized.
3.25 hrs - 3.75 hrsElbow muscle tests of the Pronator Teres, Supinator, Brachioradialis, Biceps, and Triceps are covered. Proper positioning and methods for completing the tests is emphasized.
3.75 hrs - 4 hrsOrthopedic tests for the wrist and hand including Finkelstein's Test, Phalens, Reverse Phalens, and Tinels Sign are covered. Proper positioning and methods for completing the tests is emphasized.
4 hrs - 4.25 hrsAn introduction to some basic movement patterns including glenohumeral joint rotation and scapulohumeral rythym are covered. Proper positioning and methods for completing the tests is emphasized.
